Brain Sheet Organizer

The purpose of this "brain sheet" is to provide a concise, structured way to quickly record and reference critical patient data, aiding in efficient patient care and handoffs.

The sheet includes sections for:

• Patient demographics: Name, Room, Age, Admission Date, Admitted From, Expected Discharge Date/Location.

• Medical Status: Allergy, Code Status, POLST, Past Medical History.

• Patient assessment: Pain, Activity, Mobility, Transferring, Chewing, Contacts, Hearing Aids, Dentures, Last Wound (location, size).

• Vitals: Last Set (Time, Temp, HR, RR, BP, SpO2, Pain) and Current.

• Intake & Output: Intake, Output.

• Labs: A blank section for lab results.

• Medications: Time, Rx, MOA, Side Effects, Nursing Considerations.
